How it works

Choose Your Lab Tests

You choose which lab test panel you want included with the HEALTHBOX™ Wellness Kit and virtual exam.

Test Kit Shipment

We’ll ship a Health Box Wellness Kit along with a blood pressure cuff, measuring tape and pulse ox (optional).

Virtual Visit

Your member joins a proctored call with AZOVA’s wellness advocate who will guide your members through the virtual exam including collection of vitals and blood collection. Your employee mails the test kit back to our partner laboratory.

Virtual Chronic Condition and Gaps Closure Programs

We’ll provide a comprehensive report with actionable recommendations for ongoing health. We’ll refer out to your wellness programs or you choose to use any of AZOVA’s targeted Virtual Chronic Disease Management and Gaps Closure Programs.
