Please create your account and add your name and date of birth exactly as you provided it when you scheduled your COVID vaccine appointment. You will also be asked to provide the date, time and location where you received your vaccination and a photo of your government issued photo ID and vaccination card or other record of your vaccination. If you do not provide your photo ID, we cannot create your vaccination record.
Complete the process for each person who needs to create and share a record. If you are an adult, you must create you own account and submit a request on your own behalf. If you are creating a record on behalf of a dependent, please create your own account and then add your dependent on the screen that asks who is the patient. Repeat the process for as many dependents as needed.
