Our post-COVID travel clearance service is available for travelers who have tested positive for COVID and have recovered. An independent telehealth provider will provide a written and signed travel clearance letter if you meet the requirements for travel clearance.

Requirements for Travel Clearance:

You must have a verifiable positive laboratory report for SARS-CoV-2.

A verifiable laboratory report is a test result for a RT-PCR, LAMP or rapid antigen test that was administered by a healthcare provider or that was self-administered under video observation with identity verification by an legally established telehealth service. The test that was taken must be authorized for use by the health ministry or equivalent in the country where it was purchased or administered.

Your laboratory report must have your first name, last name, date of birth, test name or type of test, administering provider or facility name or laboratory name or telehealth proctor organization name.

Your telehealth provider will review your records and will issue a signed travel clearance letter in PDF format through your AZOVA account. You will be notified that it is available via an SMS and/or email notification. You can log into your AZOVA account to view and share your PDF document.

I have provided the above information truthfully and understand and absolve AZOVA and any independent healthcare providers of any and all liability associated with this travel clearance. I understand that the current scientific literature does not specifically define a specific number of days after an individual is symptomatic or after which a positive test result is detected and that deems one as non-contagious. I understand that AZOVA and its independent health care providers who offer this service rely upon the information that I have provided and upon CDC recommendations and general clinical experience to determine if I qualify for a travel clearance.
