- Before you can purchase a COVID-19 self-collection kit, you must get a lab order prescription. Once you have completed your assessment you will be directed to choose a Walmart pharmacy location where you would like to pick up and purchase your test kit.
- Complete the lab order registration for EACH PERSON WHO IS BEING TESTED.
- We will automatically direct you back to the beginning of the assessment process for each person you are registering for.
- You can get a lab order for yourself and any dependents under your own account.
- All independent adults must create their own account and get their own lab order.
