Upload your lab results and your photo ID and submit them for confirmation.

The AZOVA records confirmation team will confirm your records and get them ready for travel clearance.

Once you have received notice that your records have been confirmed you can request your travel clearance.

If you are requesting travel clearance for a dependent, complete the process for yourself first and the system will automatically guide you to complete the process for your dependent. All independent adults must create their own account and request their own travel clearance.
Note: Please ensure your test meets the requirements for your destination. You only need a travel clearance for destinations that require a COVID test.
