How it works

STEP 1

Use the Vaxigo™ vaccination record locator tool to quickly find the location or provider where you received your COVID vaccination. If you received your vaccination from an employer or at a mass vaccination event, search for your employer name or the event location.

STEP 2
Complete a vaccination record validation request registration through AZOVA. If you received a two dose COVID vaccine and you received the doses from two different providers or locations, you will need to complete two record validation registrations. If you received both doses from the same provider or location, you only need to complete one registration. We will ask you if you need a second registration once your are done with the first.
STEP 3
AZOVA will manage your request and will attempt to locate and validate your record for you. We will provide a digital validation of your vaccination record from the actual source where you received your vaccine. If we are unable to locate and validate your record, you will receive a full refund.
STEP 4
When you arrive in [organization name], you can simply scan your QR code to share your vaccine record status. Our secure record validation system shares your vaccination record status without sharing your personal information.
