What is the AZOVA Whole Health Membership?

The Whole Health Membership is an all in one health and wellness solution for adults and children. In addition to Integrative adult and pediatric Primary Care, your membership includes access to preventive health screenings, at-home lab testing (for adults), Core Four health coaching (for adults), chronic condition management, and more. In addition, when you join AZOVA Whole Health, you have access to a connected ecosystem of specialty care providers including dermatology, psychiatry, therapy, sleep medicine with home sleep testing and DME services, clinical pharmacists, functional medicine providers, functional nutrution providers, maternal care doulas and lactation consultants and more.

You will choose an AZOVA Whole Health primary care provider as your care team leader. If you already have a primary care provider, that’s okay too! Your AZOVA Whole Health provider will still collaborate and communicate with your existing primary care and your health coach to make sure you get everything you need. You can always see your AZOVA Whole Health provider as needed for chronic condition care, prescription refills and more.

Who is the AZOVA Whole Health Membership program for?

This program is designed for adults and children who want a convenient, prevention-focused, and affordable way to manage their health.

Can I join if I already have a primary care provider?

Yes. If you already have a primary care provider, you will still choose an AZOVA Whole Health primary care provider who be your care team leader. You AZOVA Whole Health provider will collaborate with your existing primary care provider and your health coach. Your Whole Health provider is always available for chronic condition care, prescription refills and more as needed.

How does the AZOVA Whole Health Membership differ for adults and children?

Adults (ages 18-64) have access to the full suite of membership services. Pediatric members (up to 17 years old) have access to board-certified pediatric providers as well as dermatologists, psychiatrists and therapists (ages 12+ in states where available) but will not receive the HEALTHBOX Virtual Preventive Exam (must be 18+). Our annual Pediatric Wellness Visit is only available to children ages 2+.

Members-only pricing on all HEALTHBOX at-home diagnostic tests and the AZOVA Home Sleep Test
Members-only pricing on virtual specialty care, including integrated dermatology, psychiatry, therapy, sleep medicine, maternal care (including doula services, lactation consultants, and childbirth education), clinical pharmacists, functional medicine providers and more

Depending on your benefit plan, you may have no-cost or discounted rates for:
An AZOVA primary care provider or ability to include your existing provider
The Health Vitality and Prevention Program, including the HEALTHBOX Virtual Preventive Exam (comprehensive wellness lab testing + risk assessments + virtual consultation with primary care provider), at-home colon cancer screening test kit (for members over 45 years of age), and at-home prostate cancer screening test (for male members over 55 years of age)
Core Four Health Coaching focused on exercise and movement, nutrition, sleep and emotional wellness and resilience

Can I use insurance to pay for services that I receive through Whole Health?

We accept insurance for services not fully covered by your Whole Health membership. We are contracted with several health plans for these services, but you must check your financial responsibility prior to service. You can do this in one of two ways: a) contact your health plan or b) proceed with appointment registration in the AZOVA platform and check for coverage on the network participation screen. We provide medical billing services for both in-network and out-of-network members. Whether you are in-network or out-of-network, you will only be responsible for the portion of the visit fees that are not covered by your plan.

How can I determine if you accept my insurance for non-membership services?

Many of AZOVA’s non-membership services are covered by insurance or your organization. To see if we are in-network with your plan, go through the appointment registration process, choose a provider and then our system will show you if that provider is in-network or out-of-network with your plan. We will submit a claim to your plan regardless of if we are in-network or out-of-network. We will make every effort to determine what your financial responsibility will be for the visit and may ask you to pay for your portion of the visit fee prior to your visit with an AZOVA provider.

Do I have to sign up for a membership to see an AZOVA provider?

No. You can register for an appointment with an AZOVA provider at any time, with or without a Whole Health membership, through our website www.azova.com. Please note that we are not contracted with and cannot currently accept Medicare, Medicaid or Tricare patients in our practice.

Do you offer in-clinic visits?

We offer virtual visits in all 50 states and we offer in-clinic visits for members who come to our Alpine, Utah location.

What is a Care Navigator?

A Care Navigator is a member of your care team who helps you get the most out of your membership. They can help you connect with a provider, schedule appointments, help with labs and prescriptions, help with referrals, and answer any questions you may have.

How can I contact my Care Navigator?

You can access your Care Navigator 24/7 through in-app and text messaging. Just log into the AZOVA app and send a message to “azovatriage” and you’ll get taken care of!

What is Core Four Health Coaching?

Core Four Health Coaching is a program rooted in functional medicine that focuses on four key areas of health and wellness: a) exercise and movement, b) nutrition, c) sleep, and d) emotional wellness and resilience. Your health coach will work with you to develop a personalized plan to help you improve your health in each of these areas.

How do I access the no cost Virtual Urgent Care Service?

AZOVA’s virtual urgent care services are available 24/7/365 from your membership dashboard. To access the service, complete the membership enrollment and navigate to your membership dashboard. You can register for virtual urgent care at any time from anywhere.

How do I cancel my membership?

You can cancel your membership at any time by sending an email to triage@azova.com, or by sending a secure message from your AZOVA dashboard to ‘azovatriage’ requesting that your membership be canceled. Our memberships run on an annual basis and you must cancel prior to the annual renewal date. If you do cancel your membership, will have access to the Whole Health services and member discounts until the end of your membership term.
