Create an account on AZOVA and enter your voucher code to complete the registration. Upload your photo ID and a photo of your paper vaccine card or other proof of vaccination. Your electronic Vaxigo vaccine record will be dynamically created and submitted to the AZOVA vaccination records team for electronic confirmation.
The AZOVA vaccination records team will review your record and provide a confirmed status if the demographic information that you provided matches your photo ID and your vaccine card or record. If registering on behalf of a dependent, first create your own account and then add your dependent on the screen where it asks who is the patient.

Your record will be confirmed for a match between your demographic information, photo ID, vaccination card or other record and your self-reported vaccination information.
