How it works

STEP 1: Create your electronic vaccination record and request confirmation from AZOVA.

Create an account on AZOVA and enter your voucher code to complete the registration. Upload your photo ID and a photo of your paper vaccine card or other proof of vaccination. Your electronic Vaxigo vaccine record will be dynamically created and submitted to the AZOVA vaccination records team for electronic confirmation.

Step 2: Get your electronic Vaxigo vaccination record confirmed.

The AZOVA vaccination records team will review your record and provide a confirmed status if the demographic information that you provided matches your photo ID and your vaccine card or record. If registering on behalf of a dependent, first create your own account and then add your dependent on the screen where it asks who is the patient.

STEP 3: Consent to share your vaccination status with Rakuten.
AZOVA helps to manage secure sharing of vaccination information between individuals and organizations with varying levels of data sharing. Rakuten is requesting only your vaccination status information. Provide consent and we will securely share your record status with Rakuten.
STEP 4: With your consent, your vaccination record status will be shared with Rakuten automatically.
Your vaccination record status will automatically be shared when you consent for us to do so. We will also provide you with an electronic vaccination record, COVID Credentials and a free Health pass for travel. You can also share with anyone you’d like such as airlines, hotels, employers and schools at any time.
