Please be ready to provide the following when you create your account. This process will take about 3 minutes. If you are registering on behalf of a dependent, please create your own account and then add your dependent on the screen that asks who is the competitor.

• Enter your name and date of birth exactly as you provided it when you got your COVID-19 test.
• The date, time and location where you received your test.
• A photo of your government issued photo ID (passport or driver’s license).
• A photo of your test results.

Complete the process for each person who needs a Simple Health Pass. If you are an adult, you must create your own account and submit a request on your own behalf. Repeat the process for as many users as needed.
