Get a confirmed vaccination record

Please be ready to provide the following when you create your account. This process will take about 3 minutes. If you are registering on behalf of a dependent, please create your own account and then add your dependent on the screen that asks who is the competitor.

  • Enter your name and date of birth exactly as you provided it when you scheduled your COVID-19 vaccine appointment.
  • The date, time and location where you received your vaccination.
  • A photo of your government issued photo ID (passport or driver’s license).
  • A photo of your vaccination card or other valid record of your vaccination.

Complete the process for each person who will enter the facility. If you are an adult, you must create your own account and submit a request on your own behalf.

Note: You must upload your photo ID and an image of your proof of vaccination. Acceptable documents include a CDC, DOD, VAMS or vaccine provider-issued proof of vaccination, as well as the NYS Excelsior Pass, the NJ and UT Docket record, CLEAR Health Pass, and COVID Alert PA and COVID Alert CT records.
