Get your full vaccine record verified and a Simple Health Pass from AZOVA

Please be ready to provide the following when you create your account. This process will take about 3 minutes.

• Enter your name and date of birth exactly as you provided it when you scheduled your COVID-19 vaccine appointment
• The date, time and location where you received your vaccination
• A photo of your government issued photo ID
• A photo of your vaccination card or other valid record of your vaccination

Complete the process for each person who needs a Simple Health Pass. If you are an adult, you must create your own account and submit a request on your own behalf. If you are registering on behalf of a dependent, please create your own account and then add your dependent on the screen that asks who is the patient. Repeat the process for as many dependents as needed.

Note: You must upload your photo ID and an image of your proof of vaccination in order for your record verified. Acceptable documents include a CDC, DOD, VAMS or vaccine provider-issued proof of vaccination.
