How it works

Complete the vaccine eligibility screening to schedule your appointment and complete your account registration. You will receive an appointment QR code confirmation email.
Arrive at the scheduled time and present your QR code. You will be asked to present your photo ID before the vaccine is given. Then follow the instructions to get your vaccine. You may be asked to wait nearby for 15 minutes after receiving your vaccine.
After you receive your vaccination, you will receive your shareable digital vaccine record on AZOVA in addition to your CDC COVID-19 Vaccination Card.
