For adults. Requires a $300 per person annual enrollment fee. This covers the cost of the HEALTHBOX Virtual Preventive Exam Kit and associated technology and support services.
Note: Residents of New York will receive a kit without blood collection kit as it is prohibited in the state of New York. These Participants will receive a lab order for blood collection at a local laboratory.
