If you have a rash around your mouth, it could be perioral dermatitis. This condition typically appears as small, red, acne-like bumps on lighter skin or skin-colored spots on darker skin.

Perioral dermatitis typically develops around the mouth but can also appear around the eyes, nose, or occasionally in the genital area. Despite its different possible locations and names like periorificial dermatitis, it is not contagious and does not spread from person to person.

Some people with perioral dermatitis may have only a few subtle bumps, leading to a less noticeable rash. Others, however, might develop numerous lumps, resulting in a more prominent rash. Other symptoms include:

Color and texture: The bumps can vary in color, appearing skin-toned, red, or pink, depending on skin type. Though they may look like pimples, they are different from acne. The rash may appear inflamed, with redness or discoloration affecting both the rash and the surrounding skin.

Sensations: The rash can be itchy or completely itch-free, and it generally isn’t painful. Some individuals might feel a tightness or mild burning sensation, with the skin possibly becoming dry or flaky.

Granulomatous periorificial dermatitis: This type is more common in people with darker skin, who may experience less noticeable skin flushing and color changes. Burning sensations might be more frequent for those with darker skin.

There are various ways to manage perioral dermatitis, each tailored to different needs and severity levels. From over-the-counter treatments to prescription options, finding the right approach can help effectively control and alleviate the symptoms.?

  • Initial steps: Discontinue the use of facial products or cosmetics that may irritate your skin. Avoid heavy or occlusive products, as well as topical steroids, including both prescription and over-the-counter hydrocortisone. While these steroids might offer temporary relief, they often lead to worsening of the rash once stopped.
  • Over-the-counter treatments: For managing perioral dermatitis, adopt a minimal therapy approach. This means using only the gentlest products. Opt for fragrance-free, non-exfoliating face washes and moisturizers. Once the condition improves, you can gradually reintroduce your usual skincare products.?
  • Prescription medications: If over-the-counter treatments aren’t effective, consult a dermatologist or healthcare provider. They may prescribe topical or oral medications to help manage your condition.

The most common place to find perioral dermatitis is around your nose. A funny thing we’ve noticed about perioral (and periocular dermatitis) is that people who have it almost ALWAYS touch their face when they come to us and explain their condition. People who have other rashes on their face don’t seem to do this. This may be a clue the perioral dermatitis is transfered to your face by your hands. Always wash your hands before touching your face, don’t put your fingers in your nose (use a tissue) or just don’t touch your face or nose to stay safe.

Perioral Dermatitis is the most common condition that is misdiagnosed as eczema (and many other things), but it is an infection.
