What is AZOVA?

AZOVA is a 50-state provider network that provides virtual, at-home, and in-office care through a single digital network. Providers across multiple specialties offer preventive, chronic, and wellness care programs for patients, employers, payers, and retail partners.

Is AZOVA a legitimate company? Are the staff U.S.-based?

Yes, AZOVA is a legitimate and fully operational healthcare company. We provide high-quality telehealth and at-home services, working with licensed providers and verified clinical teams. While some of our support and operational staff may be located offshore, all clinical care, guidance, and health services are delivered by licensed healthcare professionals in the U.S., ensuring your care meets all applicable standards and regulations.

What types of providers can join the AZOVA Network?

AZOVA Care Group includes physicians, nurse practitioners, physician assistants, behavioral health professionals, health coaches, maternal care specialists including doulas, lactation consultants and childbirth educators who provide direct patient care or wellness services within their scope of practice.

What services does AZOVA offer?

AZOVA supports a range of care services including:

How does the contracting process work?

When you complete the “Join Our Network” lead form, our Provider Network team reviews your information and first sends you a pre-credentialing form. Our credentialing committee reviews the responses and will offer you a contract if you meet all credentialing expectations. Contracts include an Independent Contractor Agreement and a technology platform Service Order.Once signed, we’ll begin the onboarding process.

What information is required for contracting?

You’ll need to provide your current license(s), NPI, Board Certification(if applicable), DEA (if applicable), W9, ACH for you to receive payment, CAQH profile (if available),and professional and employment references

What are the credentialing requirements for licensed providers?

To participate as a licensed provider, you must maintain: An active, unrestricted license in every state where your patients are located. A valid NPI and, where applicable, prescriptive authority. Proof of malpractice history aligned with insurability requirements.Ongoing compliance with CME requirements and governing board standards.Nonlicensed providers must present and maintain their certifications, and continuing education requirements.

How long does credentialing take?

AZOVA’s internal credentialing review typically takes 1 week, depending on your responsiveness and documentation. If you are participating in payer enrollment programs, payer credentialing and enrollment timelines vary by plan and are separate from the AZOVA credentialing process.

What is the difference between AZOVA credentialing and payer credentialing (enrollment)?

AZOVA credentialing verifies your eligibility to participate on the AZOVA platform. Payer credentialing (enrollment) is required if you will bill insurance for services delivered through AZOVA. AZOVA facilitates payer enrollment applications using your existing CAQH data and supporting documents. AZOVA may obtain your CAQH information for applications but does not manage your CAQH profile on your behalf.

Will I be enrolled with health plans?

If you participate in payer-based programs, AZOVA will prepare enrollment forms and may ask you to sign credentialing documents. However, you must assist with enrollment and maintain eligibility for participation in health plans.

Which payers does AZOVA work with?

AZOVA partners with select commercial payers depending on state and specialty. You’ll be notified during onboarding which payers are active in your region and which opportunities are available for participation. It is not required to be enrolled in insurance plans as part of AZOVA.

Are there any fees to join or use the AZOVA Platform?

There are no fees to join the AZOVA network. Provider programs are “based on usage,” meaning you are paid per completed patient encounter. AZOVA does not charge annual subscription fees for contractors.

What onboarding steps should I expect?

After contracting and credentialing, you’ll complete:
AZOVA Platform setup and training
Review of service-specific protocols
Instructions for documentation, billing workflows, and charting requirements
Availability setup for your specific visits
Ongoing resources and training in the AZOVA learning center

What equipment do I need?

You must provide your own secure, reliable:Computer or smartphone, reliable High-speed internet and a Private workspace for HIPAA-compliant telehealth. AZOVA does not furnish equipment to contractors.

What are the expectations for charting?

Documentation must be:Complete and accurate including appropriate billing and diagnosis codes, Submitted same day for most visit types, Marked as “Completed” in the platform in order to be paid

What are the expectations for availability?

You set your own schedule– Just enter your availability based on when you want to be available for visits. On demand appointments will be sent to all providers and you can choose if you wish to claim the appointment. These visits are on a first-claimed-basis. For certain programs (e.g., Virtual Preventive Exams, Virtual Primary Care, and Health Coaching), you must maintain a minimum of 8 hours per week of scheduled availability to provide continuity to patients you are providing longitudinal care to.

How many clients will I be assigned?

Patient volume varies based on program demand, employer contracts, patient enrollment, and your availability. AZOVA does not guarantee a minimum number of patients. You may accept or decline patient assignments based on your schedule.

How am I paid?

You are paid per encounter. Rates vary and are listed in exhibit B of the independent contractor agreement. Payments are made monthly—on or before the 15th—for services completed in the previous month. Appointments must be completed before you can receive payment.

Who bills the patient or payer?

AZOVA handles all billing, claims, and collections. You must not bill patients or payers directly for any services delivered through the AZOVA Platform.

What happens after I submit my Provider Network Lead form?

After submission, you’ll receive an acknowledgment email. Within 3–5 business days, our Provider Recruitment team will reach out with next steps for pre-credentialing, specifics about your contract and required documentation.
