I authorize AZOVA to charge my credit card for the AZOVA Home Sleep Test. I understand that my information will be saved on file for future transactions.

The AZOVA Home Sleep Test kit must be returned after I complete my test. I will receive a free shipping label for the return. I must take my test and drop it off at a UPS Store (not a drop box) WITHIN ONE WEEK of receiving it.

If I do not return my kit to UPS within seven days of receipt, or if I return my kit with damaged or missing pieces, I authorize AZOVA to charge my credit card a $650 replacement fee.
