What is AZOVA?

AZOVA is a 50-state provider network that provides virtual, at-home, and in-office care through a single digital network. Providers across multiple specialties offer preventive, chronic, and wellness care programs for patients, employers, payers, and retail partners.

What types of providers can join the AZOVA Network?

AZOVA Care Group includes physicians, nurse practitioners, physician assistants, behavioral health professionals, health coaches, and maternal care specialists including doulas, lactation consultants, and childbirth educators who provide direct patient care or wellness services within their scope of practice.

How does the contracting process work?

When you complete the “Join Our Network” lead form, our Provider Network team reviews your information and sends you a pre-credentialing form. Our credentialing committee reviews the responses and will offer you a contract if you meet all credentialing expectations. Contracts include an Independent Contractor Agreement and a technology platform Service Order. Once signed, we’ll begin the onboarding process.

What information is required for contracting?

You’ll need to provide your current license(s), NPI, Board Certification (if applicable), DEA (if applicable), W9, ACH for you to receive payment, CAQH profile (if available), and professional and employment references.

How long does credentialing take?

Accepted Answer

AZOVA’s internal credentialing review typically takes 1 week, depending on your responsiveness and documentation. If you are participating in payer enrollment programs, payer credentialing and enrollment timelines vary by plan and are separate from the AZOVA credentialing process.

Question 6

Are there any fees to join or use the AZOVA Platform?

There are no fees to join the AZOVA network. Provider programs are based on usage, meaning you are paid per completed patient encounter. AZOVA does not charge annual subscription fees for contractors.

Question 7

What are the expectations for availability?

Accepted Answer

You set your own schedule — just enter your availability based on when you want to be available for visits. On-demand appointments are sent to all providers and you can choose to claim them on a first-claimed basis. For certain programs (e.g., Virtual Preventive Exams, Virtual Primary Care, and Health Coaching), you must maintain a minimum of 8 hours per week of scheduled availability to provide continuity to patients you care for longitudinally.

How am I paid?

Accepted Answer

You are paid per encounter. Rates vary and are listed in your independent contractor agreement. Payments are made monthly — on or before the 15th — for services completed in the previous month. AZOVA handles all billing, claims, and collections.

What happens after I submit my Provider Network Lead form?

Accepted Answer

After submission, you’ll receive an acknowledgment email. Within 3–5 business days, our Provider Recruitment team will reach out with next steps for pre-credentialing, specifics about your contract, and required documentation.
