Select adult or pediatric care, then choose an e-visit or an in-office appointment. The options shown are designed to route you toward the right registration and scheduling experience.
Step 2
Register securely
Create or access your AZOVA account and provide contact, location, health-history, and visit information. For a child, a parent or legal guardian should complete the appropriate registration details.
Step 3
Complete your visit
Connect virtually or arrive at the selected office as instructed. Be ready to discuss symptoms, timing, medications, allergies, relevant history, and what you hope to understand or accomplish.
Step 4
Follow your care plan
Your clinician explains recommended next steps. Depending on the concern, your plan may include supportive nutrition and lifestyle guidance, treatment, monitoring, follow-up, or a referral for another level of care.

What is the difference between an e-visit and in-office care?
An e-visit lets you connect without traveling and is suitable for concerns that can be evaluated remotely. In-office care allows an in-person examination. Choose from the available options, and seek urgent local care for severe or emergency symptoms.
Can I book for my child?
Yes. Select the pediatric option and complete registration as the parent or legal guardian. Age, location, and the nature of the concern may affect which visit formats are available.
What should I have ready?
Have an up-to-date medication and allergy list, relevant health history, a brief timeline of symptoms, and any questions you want answered. For a virtual visit, use a private place with a stable internet connection.
Is this service for emergencies?
No. If symptoms are severe, rapidly worsening, or potentially life-threatening, call 911 or go to the nearest emergency department rather than waiting for a primary-care appointment.
